Call to cancel your appointment as soon as you know you won’t be able to make it and no later than 24 hours before your appointment. Patients who miss three appointments in a row will have their record reviewed before being able to make another appointment.

Via Care offers a sliding fee discount schedule to all patients for all of our services. To obtain more information, please call our front desk at (323) 268-9191.

 

What To Bring To Your First Appointment

If this is your first appointment, please bring the following to determine your eligibility on the sliding fee scale:

  • Proof of Income (paystub, unemployment check, letter from Social Security, tax return)

  • Proof of address in Los Angeles County (utility bill, lease agreement, CA driver’s license, or ID)

  • Photo identification (California driver’s license, California identification card, and ID from your country of origin, the Matricular Consular ID from Mexico, a passport and school ID). Please remember, you will not be denied services based on your income or citizenship status.

Enroll in Health Coverage

Via Care accepts all patients regardless of insurance availability or financial status. Our federally funded health care program allows us to offer a wide range of services to patients through a sliding-fee scale; we also take Medi-Cal and various forms of insurance. Programs​

 

What to Bring if You’re Applying for Health Coverage

 

For all programs, you will need:

  • Proof of income (pay stub, unemployment insurance stub, income tax return, Social Security award letter, a letter from your employer if you are paid in cash*)

  • Government-issued Photo ID (State ID, Driver License, Passport, Military ID card, etc.)

  • Proof of address (such as a utility bill with your name on it, or a driver license*)

 

If you have them, you should also bring:

  • Birth certificate

  • US residency card

  • Automobile registration

  • Social Security card

  • Proof of pregnancy

 

You can also issue an affidavit if you have no other proof of income or address.

If you are not a U.S. citizen, you are still encouraged to bring in proof of income and address verification as you may be eligible for a variety of no-cost or low-cost health care programs. These documents are used solely for determining your eligibility for health care programs and will NOT be submitted to the US Citizenship and Immigration Service (USCIS) or Internal Revenue Service (IRS).

 

Do you qualify for Medi-Cal?
 

Even if you’ve been turned down in the past, the Affordable Care Act has made more people eligible for Medi-Cal. You may be eligible if:​

  • Your income is less than $1,677 for a family of one (check here for all family sizes)

  • You are pregnant

  • You are blind or disabled

  • You are already enrolled in Cal-Fresh, Cal-WORKs, SSI/SSP, Refugee Assistance, or Foster Care/Adoption Assistance Program

 

There are other circumstances where people qualify for Medi-Cal. It’s time to check!

 

Curious about Covered California?
 

Our enrollers are trained and certified to help you navigate the options. You may be eligible for subsidized private health insurance if you make less than $4,860 per month (for a family of one), and you can always enroll regardless of income. Don’t pay a penalty to the IRS… pay for the health insurance you deserved instead!

A trained nurse will answer your health care questions so you can take care of yourself or your family as soon as possible. The nurses will help you in your language at no cost to you. Looking for a ride to the clinic? Check out the options below to fit your needs:

  • For your convenience, Via Care provides transportation.  Please call the clinic 3 days in advance before your appointment to make arrangements.  Call 323-268-9191.

 

  • If you have a medical need*, you’re qualified to get free rides to and from your doctor. To plan a ride, call at least 48 hours before your office visit. The ride is free but is not for emergencies. When you call, have your health ID card, your doctor’s name, phone and street address. * Medi-Cal enrollees must have a medical necessity form from your doctor on file with LogistiCare that tells why you need a ride to the doctor.

  • The El Sol Shuttle Service covers East LA with 3 lines. The fare is only 25 cents and seniors (60 years and older), persons with disabilities, children under 5, and Metro Pass Holders ride free!

Via Care complies with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Via Care protects confidential health care information, known as “Protected Health Information” (PHI).

Via Care may use and give your health information to:

  • Treat you

  • Get paid for your treatment

  • Operate health care services

 

Via Care may use and give your health information for:

  • Law enforcement requests

  • Judicial and administrative proceedings related to legal actions

  • Healthcare fraud and abuse detection or compliance with the law

  • Use by another healthcare provider treating you

  • Government health oversight activities

  • Reports required by law related to births, deaths or diseases

  • Reports required by law related to neglect and abuse, or domestic violence

  • Notifying a party about exposure to a possible communicable disease

  • Use by another healthcare provider for payment to that provider

  • Military, national defense and security or other governmental functions

  • Workers’ compensation purposes and in compliance with related laws

  • Averting a serious threat to public health and safety

 

 You have the right to:

  • Inspect or get a copy of your medical record

  • Change information on your medical record if you think it is incorrect

  • Get a list of persons with whom Via Care shared your PHI

  • Ask Via Care to limit the information it shares

  • Ask for a copy of your privacy notice

  • Write a letter of complaint to Via Care Community Health Center or the federal government

Patient Rights & Responsibilities

As a patient, you have the right to:

  • Take part in your health care and treatment

  • Know the names of the people caring for you

  • Be treated with respect and dignity in a safe and private setting

  • Be informed about your illness and treatment, including options for your care

  • Change medical providers at Via Care

  • Get another opinion about your illness or treatment

  • Privacy of your health records

  • Talk with the clinic manager about any questions or problems with your care

  • Know about services available through Via Care

  • Respect for your cultural, social, spiritual and personal values and beliefs

  • Know about legal reporting requirements

  • Ask for special arrangements if you have a disability

  • Ask for help with a living will or durable power of attorney for health care

  • Refuse treatment, care and services as allowed by law

  • Know the cost of your care and ways you may pay for your care

  • Refuse to be included in any research program without limiting medical care or treatment

 

As a patient, you have the responsibility to:

  • Tell your medical provider about your illness or problems

  • Ask questions about your illness or care

  • Show respect to both care givers and other patients

  • Cancel or reschedule appointments so that another person may have that time slot

  • Pay your bills on time

  • Use medications or medical devices for yourself only

  • Inform the medical provider if you become worse or you have an unexpected reaction to a medication

  • Give written permission to release your other health records to Via Care, when necessary

  • Provide Via Care a copy of your living will or durable power of attorney for healthcare matters
