ENROLLMENT SERVICES

Via Care accepts all patients regardless of their insurance or financial status. Via Care accepts and offers enrollment assistance for the following forms of insurance:​

  • Medi-Cal

  • Medi-Care

  • Medi-Medi

  • Child Health & Disability Prevention (CHDP)

  • Family PACT

  • Private Insurance

Via Care offers a sliding fee discount schedule to all patients for all of our services. Eligibility is based on family size and income. Below are links to our sliding fee schedule and application:

ATTENTION: An upcoming California law allows all eligible California residents to get free full-scope Medi-Cal regardless of immigration status on January 1, 2024! Apply today to be automatically enrolled in full-scope Medi-Cal on January 1, 2024.

Via Care offers enrollment assistance for the following forms of insurance:​

What to Bring if You're Applying for Health Coverage

For all programs, you will need:

​

  • Proof of income (pay stub, unemployment insurance stub, income tax return, Social Security award letter, a letter from your employer if you are paid in cash*)

  • Government-issued Photo ID (State ID, Driver License, Passport, Military ID card, etc.)

  • Proof of address (such as a utility bill with your name on it, or a driver license*

 

If you have them, you should also bring:

​

  • Birth certificate

  • US residency card

  • Automobile registration

  • Social Security card

  • Proof of pregnancy

 

* For My Health LA only, you can also issue an affidavit if you have no other proof of income or address.

If you are not a U.S. citizen, you are still encouraged to bring in proof of income and address verification as you may be eligible for a variety of no-cost or low-cost health care programs.

These documents are used solely for determining your eligibility for health care programs and will NOT be submitted to the US Citizenship and Immigration Service (USCIS) or Internal Revenue Service (IRS).
