Via Care’s full-service pharmacy offers a wide range of medications and clinical pharmacy services for our patients, including specialty pharmacy services for Hepatitis C and HIV prevention (PrEP/PEP).

The Via Care pharmacy provides low-cost medications to our patients. An in-house pharmacy brings more accessibility for our patients so they can get their prescriptions on time at their own community clinic in East Los Angeles.

Via Care Clinical Pharmacy Services

Chronic Disease Management services include one-on-one appointments with a Via Care pharmacist to assist patients to better understand their chronic condition, instructions on medication use, and how to improve their health through lifestyle changes.

 

Medication Management services include a personalized care program for each patient developed by a Via Care pharmacist to ensure the proper use of prescribed medications and to prevent adverse drug events.

 

Diabetes Education offered by a Via Care pharmacist includes individual and group education on nutrition, exercise, self-monitoring, and medication adherence to improve overall health and reduce diabetes-related complications such as foot ulcers, vision impairment, heart disease, and kidney disease.

Specialty Pharmacy includes medications and medical services to treat Hepatitis C and prevent HIV (PrEP/PEP). Patients taking medications for Hepatitis C and HIV prevention have access to our concierge case management program.

Mahsa Dadkhah, PharmD, APh is a director of pharmacy at Via Care Community Health Center. She graduated with a bachelor of science in biology from University of California, Irvine and received her doctor of pharmacy degree at Western University of Health Sciences. She was employed as an ambulatory care pharmacist at Clinica Mrs. Oscar Romero where she had extensive experience in anticoagulation, diabetes management, hypertension and dyslipidemia managements.

 

Dr. Dadkhah is a community-based pharmacy practitioner who is licensed as an advanced practice pharmacist. Dr. Dadkhah's practice site includes the community-based, collaborative practice clinic with the all medical providers. Her practice interests include: medication therapy management (MTM), comprehensive medication management (CMM), community-based pharmacy practice, advanced pharmacy practice, travel health, immunizations, community health, and disease screening, management, and prevention.
